Role:

We are seeking a dedicated Registered Nurse (RN) – Case Manager to join our interdisciplinary care team. This role is pivotal in ensuring seamless care coordination, documentation accuracy, and collaboration with healthcare providers to optimize patient outcomes. The RN will work closely with advanced practice clinicians (APCs) to support patient care, facilitate therapy coordination, and ensure compliance with quality measures.

Key Responsibilities:

Maintain precise and comprehensive documentation of patient records. Collaborate with the interdisciplinary care team to enhance patient care. Assist providers with various care coordination activities. Support benefit determinations related to Medicare Part A, B, and D. Identify and respond to patient condition changes in collaboration with APCs and facilities. Facilitate therapy coordination and ensure smooth transitions for patients. Participate in facility-based meetings, team conferences, and case presentations. Coordinate and conduct training sessions and in-services for selected audiences. Assist providers with accurate documentation, coding, and medication reconciliation. Write verbal orders from providers, adhering to state RN practice laws and facility regulations. Manage patients experiencing acute medical situations or chronic condition exacerbations. Assist in short-term sub-acute rehabilitation episodes.

Qualifications & Requirements:
Required:

Active and unrestricted RN license in the state of residence (or ability to obtain by start date). Associate degree or higher in Nursing. Basic Life Support (BLS) certification. Ability to lift up to 30 pounds and perform necessary physical assessments. Reliable transportation for facility visits. Full

vaccination (compliance with all federal, state, and local regulations). Successful completion of a drug test (UnitedHealth Group is a drug-free workplace).

Preferred:

Minimum 1 year of clinical experience (long-term care setting preferred). Expertise in managing patients with complex behavioral, social, or functional needs. Strong understanding of geriatrics and chronic illnesses. Proficiency in electronic medical records and documentation. Effective communication, time management, and collaboration skills. Ability to travel between seven designated facilities.
