Position Summary:

The Hospice Homecare Nurse makes visits and provides hospice care in patients homes and facilities and plans, organizes, and directs the hospice home care services to support patients living with a life-limiting illness. The hospice nurse is responsible for direct patient care, making visits in homes and facilities, by assessing needs and developing and implementing an individualized plan of care that maintains the highest level of comfort and quality of life. The Hospice Nurse coordinates and supervises the care using an interdisciplinary team approach. In addition, the Hospice Nurse performs ongoing assessment and revises the initial written plan of care with team collaboration as the needs and conditions of the patient/family change.
