PURPOSE AND SCOPE

The registered professional nurse (CAP RN 1)position is an entry level designation into the ClinicalAdvancement Program for Registered Nurses. The

CAP RN 1

isaccountable and responsible for the provision and coordination ofclinically competent care including assessment, planning,intervention and evaluation for an assigned group of patients. As amember of the Chronic Kidney Disease (CKD) health care team, the

CAP RN 1

participates in decision-making, teaching, leadershipfunctions, and quality improvement activities that enhance patientcare outcomes and facility operations.

PRINCIPAL DUTIES ANDRESPONSIBILITIES

All duties and responsibilities are expected tobe performed in accordance with Fresenius Kidney Care policy,procedures, standards of nursing practice, state and federalregulations.

  • Performs all essential functions under the directionof the Clinical Manager (CM) and with the guidance from theEducator, Preceptor or a CAP RN 2, 3, 4 or 5 for the first 6 monthsof employment.
  • Expected to complete requirements to advance tothe next CAP level, for which they qualify, within 24 months fromdate of hire.
  • Performs ongoing, systematic collection andanalysis of patient data pre – during – post hemodialysis treatmentfor assigned patients and documents in the patient medical record,adjusts or modifies the treatment plan as indicated and notifiesTeam Leader, Charge Nurse, Clinical Manager or Physician as needed.
  • Recognizes aspects and implications of patient status that varyfrom normal and reports to or collaborates with appropriate healthteam members for input.
  • Assesses daily assigned patient careneeds and collaborates with direct and ancillary patient care staffas needed.
  • Directs and provides, in collaboration with direct andancillary patient care staff, all aspects of the daily provision ofsafe and effective delivery of chronic hemodialysis therapy toassigned patients.
  • Administers medications as prescribed or inaccordance with approved algorithm(s), and documents appropriatemedical justification and effectiveness.
  • Ensures correctlaboratory collection, processing and shipping procedures areperformed and reschedules missed or insufficient laboratorycollections.
  • Ensures physician orders for assigned patients areentered and acknowledged in the Medical Record
  • Initiates orassists with emergency response measures.
  • Delegates appropriatetasks to direct patient care staff including but not limited toLVN/LPNs and Patient Care Technicians.
  • Monitors patient carestaff for appropriate techniques and adherence to FKC policy andprocedures.
  • Promotes infection control, equipment andenvironmental safety.
  • Assesses, collaborates and documentspatient/family’s basic learning needs to provide initial andongoing education to patients and family.
  • Identifies expectedoutcomes, documents and updates the nursing assessment and plan ofcare for assigned patients through collaboration with theInterdisciplinary Team.
  • Ensures patient awareness related totransplant and treatment modality options.
  • Makes referrals toSocial Worker and Registered Dietitian as appropriate forindividual patient education and counseling.
  • Checks availabilityof adequate emergency equipment and supplies as assigned.
  • Assistswith medication tracking and inventory as assigned.
  • Serves as aresource for health care team, participates in staff training andorientation of new staff as assigned.
  • Participates in allrequired staff meetings as scheduled.
  • Supports and promotes theFMCNA’s mission, vision, values, and customer service philosophy.
  • Performs all other duties as assigned by supervisor.
PHYSICALDEMANDS AND WORKING CONDITIONS

  • The position provides direct patient care thatregularly involves heavy lifting and moving of patients, andassisting with ambulation.

Equipment aids and/or coworkers mayprovide assistance. This position requires frequent, prolongedperiods of standing and the employee must be able to bend over. Theemployee may occasionally be required to move, with assistance,machines and equipment of up to 200 lbs., and may lift chemical andwater solutions of up to 30 lbs. as high as 5 feet. The workenvironment is characteristic of a health care facility with airtemperature control and moderate noise levels. May be exposed toinfectious and contagious diseases/materials.

  • Day to day workincludes desk work, computer work and interaction with patients,facility/hospital staff and physicians. The position may requiretravel to training sites or other facilities.
  • May be asked toprovide essential functions of this position in other locationswith the same physical demands and working conditions as describedabove.
SUPERVISION
  • Assigned oversight of Patient CareTechnicians/LPNs as a Team Leader or designated Nurse in charge,after the following:
  • Successful completion of all FKC educationand training requirements for new employees plus
  • Must have aminimum of 9 months experience as an RN plus
  • Successfulcompletion of 3 months experience in chronic hemodialysis.
EDUCATION
  • G raduate of an accredited School of Nursing.
  • Current appropriate state licensure.
  • Must meet the practicerequirements in the state in which he or she is employed.
EXPERIENCE AND REQUIRED SKILLS
  • Entry level for RNs with lessthan 2 years of nephrology nursing experience within the last twoyears or for RNs with a BSN and less than 1 year of nephrologynursing experience within the last two years.
  • Minimum of 9 monthsexperience as a Registered Nurse (preferred).
  • Chronichemodialysis experience (preferred).
  • Successful completion of FKCEducation and Training requirements for new employees.
  • Successfulcompletion and maintenance of CPR BLS Certification.
  • Meet allconditions of employment including Ishihara’s Color Blindness Test.
  • Successful completion of all annual requirededucation/training/competencies.
  • Successful completion ofon-going training and education as assigned or required.
