Hybrid schedule, 2 days remote, 3 days onsite -OccSummaryThe Oncology Nurse Navigator (ONN) is a professional RN withoncology- specific knowledge who offers personalized assistance topatients, families, and caregivers to address health systembarriers. For patients with barriers, health-related social needs,or other factors that contribute to risk of untimely or fragmentedcare, the ONN assesses for and addresses barriers, provides patientwith personalized education and support, and communicates withother members of care team to support patient receiving timely,quality care (medical and psychosocial care). Due to the complex,multi-disciplinary, multi-site care model of cancer care, theoncology nurse navigator has specific clinical knowledge of thecare, the other treatment providers, and the disease process thatis critical to support patients and families across the continuum,from prevention and screening to survivorship or end of life. TheONN builds and maintains trusting relationships with patients andcaregivers, andcollaborators in the cancer care continuum(community resources, primary care providers, cancer careproviders, etc., as appropriate) to support communication,defragmentation, and seamless, timely care. -Work Performed -ForOncology patients with barriers or health-related social needs, orother factors that contribute to risk of untimely or fragmentedcare, the ONN identifies & addresses barriers and communicates withother members of care team to support patients. ForOncologypatients with highly acute care needs, such as complextreatment plans, multiple co-morbidities, etc., the ONN providesfocused education and support, addresses barriers, and communicateswith other members of the care team to support patient on cancercontinuum. Provide communication, advocacy, and education in aculturally competent manner. Ability to clinically triageidentified complex patient cases (those with significant barriersand health-related social needs or otherwise identified as avulnerable population within oncology) and navigation referralsefficiently to ensure timely, equitable access to care. Collaboratewith multidisciplinary care team (including cancer specialists,primary care, and other specialists) to facilitate shared decisionmaking and engagement of patients in their care. Provide educationand reinforcement of clinical information to oncology patients andcaregivers about screenings, diagnosis, treatment, survivorship,end of life, psychosocial care, other specialty care, asneeded/appropriate. Focused symptom assessment and management foridentified patient population in oncology. Assess physical andpsychosocial needs for identified patient population and provideeducation, referrals for services (medical family therapists,clinical social workers, physical and occupational therapists,dietitians, etc.), and communication with collaborating clinicianson care for patients. Assess health literacy and educational needsof oncology patient and family caregivers. Providepersonalizededucation to patient and family members, and coordinate with careteam to ensure any barriers are identified and addressed. Assessand document patient’s needs, goals, and preferences and work toensure such are integrated into treatment and care delivery.
Identify gaps in, or fragmented care, and facilitate reduction orelimination of barriers to seamless, timely care through patient orsystem-focused interventions Enhance access to oncology clinicaltrials and optimize communication with patients, families andinvestigators regarding clinical trials, including personalizededucation for patients about clinical trials, build trustingrelationships in relation to clinical trials, and support thepatients in shared decision making regarding participation inclinical trials. Monitor identified patient population’sparticipation in care (i.e. cancellations, no shows, adherence totreatment plans) and collaborate with patient to identifybarriers/needs and with other clinicians on strategies, in order tohelp patients with adherence to care or possible adaptations tocare. Educate multidisciplinary colleagues about the role of theONN, identified barriers, available resources, and the navigationprocess. Report navigation program outcomes to key stakeholdersPossess leadership skills including staff education, preceptorship,patient education, and participate in process improvementinitiatives. Practice according to professional and legalstandards. Maintain navigation services documentation according toprogram standards. -Knowledge, Skills and Abilities -Excellentwritten, oral and conversational communication skills toeffectively work with diverse groups. Ability to analyze, evaluatemultiple solutions and solve complex problems using well developedcritical & analytical thinking skills. Excellent time managementskills needed to prioritize among many competing priorities.

Metric tracking skills -Minimum Qualifications – -EducationBSNrequired. MSN preferred. -Experience -Three years of experience ofspecialty specific nursing in a hospital, clinic, or communitysetting. Experience in nurse navigation preferred Completion ofnational nurse navigation training preferred. Navigation trainingor credential to be completed within 2 years of hire. -Degrees,Licensures, Certifications -Must have current or compact RNlicensure in the state of North Carolina. BLS required.
