Occ Summary The Oncology Nurse Navigator (ONN) isa professional RN with oncology- specific knowledge who offerspersonalized assistance to patients, families, and caregivers toaddress health system barriers. For patients with barriers,health-related social needs, or other factors that contribute torisk of untimely or fragmented care, the ONN assesses for andaddresses barriers, provides patient with personalized educationand support, and communicates with other members of care team tosupport patient receiving timely, quality care (medical andpsychosocial care). Due to the complex, multi-disciplinary,multi-site care model of cancer care, the oncology nurse navigatorhas specific clinical knowledge of the care, the other treatmentproviders, and the disease process that is critical to supportpatients and families across the continuum, from prevention andscreening to survivorship or end of life. The ONN builds andmaintains trusting relationships with patients and caregivers,andcollaborators in the cancer care continuum (community resources,primary care providers, cancer care providers, etc., asappropriate) to support communication, defragmentation, andseamless, timely care. Work Performed For Oncology patients with barriers orhealth-related social needs, or other factors that contribute torisk of untimely or fragmented care, the ONN identifies & addressesbarriers and communicates with other members of care team tosupport patients. For Oncologypatients with highly acute careneeds, such as complex treatment plans, multiple co-morbidities,etc., the ONN provides focused education and support, addressesbarriers, and communicates with other members of the care team tosupport patient on cancer continuum. Provide communication,advocacy, and education in a culturally competent manner. Abilityto clinically triage identified complex patient cases (those withsignificant barriers and health-related social needs or otherwiseidentified as a vulnerable population within oncology) andnavigation referrals efficiently to ensure timely, equitable accessto care. Collaborate with multidisciplinary care team (includingcancer specialists, primary care, and other specialists) tofacilitate shared decision making and engagement of patients intheir care. Provide education and reinforcement of clinicalinformation to oncology patients and caregivers about screenings,diagnosis, treatment, survivorship, end of life, psychosocial care,other specialty care, as needed/appropriate. Focused symptomassessment and management for identified patient population inoncology. Assess physical and psychosocial needs for identifiedpatient population and provide education, referrals for services(medical family therapists, clinical social workers, physical andoccupational therapists, dietitians, etc.), and communication withcollaborating clinicians on care for patients. Assess healthliteracy and educational needs of oncology patient and familycaregivers. Providepersonalized education to patient and familymembers, and coordinate with care team to ensure any barriers areidentified and addressed. Assess and document patient’s needs,goals, and preferences and work to ensure such are integrated intotreatment and care delivery. Identify gaps in, or fragmented care,and facilitate reduction or elimination of barriers to seamless,timely care through patient or system-focused interventions Enhanceaccess to oncology clinical trials and optimize communication withpatients, families and investigators regarding clinical trials,including personalized education for patients about clinicaltrials, build trusting relationships in relation to clinicaltrials, and support the patients in shared decision makingregarding participation in clinical trials. Monitor identifiedpatient population’s participation in care (i.e. cancellations, noshows, adherence to treatment plans) and collaborate with patientto identify barriers/needs and with other clinicians on strategies,in order to help patients with adherence to care or possibleadaptations to care. Educate multidisciplinary colleagues about therole of the ONN, identified barriers, available resources, and thenavigation process. Report navigation program outcomes to keystakeholders Possess leadership skills including staff education,preceptorship, patient education, and participate in processimprovement initiatives. Practice according to professional andlegal standards. Maintain navigation services documentationaccording to program standards. Knowledge, Skills and Abilities Excellent written, oral andconversational communication skills to effectively work withdiverse groups. Ability to analyze, evaluate multiple solutions andsolve complex problems using well developed critical & analyticalthinking skills. Excellent time management skills needed toprioritize among many competing priorities. Attention to detailneeded to operate accurately and effectively. Mastery of MicrosoftOffice, especially Word and Excel, and use of health record fordocumentation and/or reporting. Metric trackingskills Minimum Qualifications Education BSN required. MSN preferred. Experience Three years of experience of specialty specific nursingin a hospital, clinic, or community setting. Experience in nursenavigation preferred Completion of national nurse navigationtraining preferred. Navigation training or credential to becompleted within 2 years of hire. Degrees, Licensures, Certifications Must have current or compact RNlicensure in the state of North Carolina. BLS required.
