As a Hospice RN/ RN Case Manager, you will: Report directly to the Administrator, Executive Director orPatient Care Manager Senior and will maintain primary control andprofessional management of each patient. Act as primary liaison between physician, patient/family andhospice team. Provide direct patient care services to hospice patients andsupportive care to the patient’s family with a reasonable case loadwhile providing a trusting and compassionate relationship with yourpatients and their families. Empower patient’s caregivers by educating them on care fortheir loved one Manage and collaborate with an interdisciplinary groupconsisting of Licensed Practical/Vocational Nurses (LPN or LVN),Certified Nursing Assistants (CNA) and support staff to providecomfort, peace, and dignity Evaluate patient and family needs and assist in the preparationand execution of the plan of care for each patient. Maintain all assigned patient clinical records. Participate in company quality assessment and performanceimprovement programs.

RN CaseManager Requirements:

Hospice heart: compassionate, kind and empathetic to yourpatients and their families. Comfortable providing care wherever the patient calls”home”. Demonstrate flexibility and composure while supporting othersin a time of crisis. RN license in the state of requested employment (or eligibilityto obtain). 1+ years of nursing experience in hospice, home health, ICU,geriatric, or oncology strongly preferred. Additional, preferred experience to help you be successful in ahospice role: hospital Staff RN, Med Surg, PACU, Float pool, ER oremergency room, critical care, acute care, cardiac, geriatric,admissions, wound care, telemetry or travel nursing.
