An Excellent Opportunity Mountain Valley has an opening for a Registered Nurse Liaison who will work in the Winston-Salem service areas in the 2 major hospital systems. As a hospice liaison you will have the responsibility for responding to hospice referrals at the hospitals, preparing with our care teams for services needed, and providing a physical assessment and admission to hospice as required. Duties Maintain a hospice presence in the area hospitals Acts as a partner in care with physicians, case managers and other facility staff. Educate patients and families about the hospice benefit and services. Provides an initial physical assessment and admission to hospice as required. Initiate the referral to admission process through gathering information, education, and form completion aspects of patient intake, admissions, and transition to home. Perform hospice in place (hospital) admissions as needed. Qualifications Valid Registered Nurse License BLS Certification Valid Driver’s License Schedule Monday

  • Friday 8am
