NegotiablePrimary Responsibilities Ensure accurate and complete documentation. Communicate and collaborate effectively with theinterdisciplinary care team. Support providers and care teams in various care coordinationactivities. Assist in benefit determination related to Medicare Part A,Part B, and Part D. Collaborate with the primary advanced practice clinician (APC)and facility to address patient condition changes. Facilitate therapy coordination and ensure seamless integrationinto patient care plans. Participate in facility-based meetings to align carestrategies. Schedule and participate in family conferences, team meetings,and case presentations. Assist with training coordination, quality measure tracking,and compliance. Develop and lead in-service training sessions for selectedaudiences. Support providers in maintaining accurate documentation,coding, medication reconciliation, nursing rounds, assessment, andpatient records. Comply with state RN practice laws and facility protocols whenwriting verbal orders. Aid in managing acute medical situations and chronic conditionexacerbations, including diagnostics, testing, andinterventions. Assist members throughout short-term sub-acute rehabilitationepisodes.

Required Qualifications Active and unrestricted RN license in the state of residence(or ability to obtain by start date). Associate degree or higher in Nursing (RN). Basic Life Support (BLS) certification. Ability to perform physical tasks, including lifting a 30-poundbag and conducting physical assessments. Reliable access to transportation. Full

PreferredQualifications 1+ year of clinical experience (long-term care experiencepreferred). Experience caring for patients with complex behavioral, social,or functional needs. Strong understanding of geriatrics and chronic illnessmanagement. Proficiency in medical documentation, both written andelectronic. Excellent communication, time management, and teamworkskills. Ability to work collaboratively across diverse caresettings. Reliable transportation and ability to travel betweenfacilities.
