Working closely with a dedicated team of physicians and clinicians,you’ll develop and manage care plans that support recovery and helppatients get back to the life they love. (Sat-Mon) The ClinicalAssessment Advisor directly completes and oversees development/ ofthe plan of treatment as approved by physician, performs ongoingevaluation of patient needs and coordinates team of professionalsand other licensed clinicians and home health aides to ensureoptimal clinical outcomes. Works with physician and/or referralsource to obtain all pertinent clinical information for optimalassessment generation. Ensures consistent team care scheduling anddissemination of updated clinical information. Coordinates assignedcare team members and resources. Maintains focus on all healthcarequality and affordability initiatives (HCQAI’s). Establishedproductivity standards and performance will be monitored andmeasured under general supervision of the Clinical Field StaffSupervisor (CFSS) or above.

Essential Functions:
  • Performs ordelegates the initial and ongoing evaluation of patient needswithin their scope of practice
  • Routinely complete Start ofCare/Resumption of Care/Recertification/Discharge of OASIS visitsand may occasionally need to provide routine visits based onindividual patient needs and branch expectations
  • Ensures plan ofcare incorporates and guides appropriate teaching related to healthmaintenance, prevention and safety
  • Coordinates availableresources to manage care plan and ensures stated outcomes areachieved
  • Periodically reassesses or delegates the reassessment ofpatient needs and revises care plan as necessary
  • Assuresappropriate care of patient is met through the start of careassessment completion.
  • Promotes/coordinates communication betweenteam members, attending physicians, appropriate administrativestaff, referral sources and external case managers to ensureappropriateness of care
  • coordination and communicates anynecessary changes to the plan of care
  • Ensures patient needs areidentified and qualified caregivers are assigned to all caseswithin their scope of practice
  • Participates in special projectsand performs other duties as assigned Use your skills to make animpact
Required Experience/Skills:
  • Bachelor of Science degree inNursing (BSN) preferred
  • At least 2 years clinical home careexperience with minimum one year experience in a clinicalmanagement role is preferred
  • Current CPR certification
  • Excellent verbal and written communication skills
  • Excellentinterpersonal skills
  • Knowledge of state and federal home healthagency regulations and compliance standards and regulations
  • Knowledge of clinical policies and procedures and ability toimplement
  • Knowledge of clinical structure of PDGM
  • Must read,write and speak fluent English.
  • Must have good and regularattendance.
  • Approximate percent of time required to travel: 60%
  • Performs other related duties as assigned.
  • A valid driver’slicense, auto insurance, and reliable transportation are required.

Pay Range

  • $45.00
  • $63.00
  • pay per visit/unit
  • $70,500
  • $96,900 per year base pay Scheduled Weekly Hours 40 Pay Range Thecompensation range below reflects a good faith estimate of startingbase pay for full time (40 hours per week) employment at the timeof posting.

The pay range may be higher or lower based ongeographic location and individual pay will vary based ondemonstrated job related skills, knowledge, experience, education,certifications, etc. $70,500
