Position Summary:
The Wound Care/Admissions Nurse plays a key role in both the care and welcoming of new residents to our community.
This full-time position includes:
Admissions Responsibilities (60% of role): o Greet and welcome new residents as they move in, ensuring a smooth and welcoming transition into the community. o Conduct comprehensive assessments of new residents to gather medical and personal history to assess care needs. o Meet with new residents and their families to introduce the facility, explaining services and community offerings. o Ensure all required documentation is completed accurately and promptly, including consent forms, medical history, and insurance paperwork. o Collaborate with other departments to prepare the resident’s room and ensure they have all necessary accommodations. o Serve as the primary point of contact for incoming residents, addressing questions and concerns regarding their transition. o Coordinate with IDT to ensure a seamless and positive admission experience. Wound Care Responsibilities (40% of role): o Assess and treat residents with wounds, pressure ulcers, and other skin conditions. o Develop individualized wound care plans for residents, in consultation with physicians and other healthcare providers. o Regularly monitor and document wound healing progress, adjusting care plans as necessary. o Administer wound care treatments, including dressing changes and debridement procedures. o Educate residents and their families on wound care best practices to encourage healing and prevent further complications. o Maintain accurate and thorough medical records related to wound care treatments and progress. o Stay current on wound care best practices, techniques, and new developments.
Qualifications:
Licensed Registered Nurse (RN) with current, active state licensure. At least 3-4 years of experience in nursing, preferably in long-term care, rehabilitation, or a similar healthcare setting. Wound care certification is a plus (e.g., CWCN, WCC, or other related certifications). Strong clinical skills with experience in wound care management and treatment. Excellent communication and interpersonal skills, with the ability to work well with residents, families, and staff members. Strong organizational skills and attention to detail, with the ability to manage multiple tasks simultaneously. Ability to maintain confidentiality and adhere to HIPAA guidelines. Compassionate, patient-centered approach to care.
