The Congregational Nurse supports the broader community byaddressing disparities in health and wellness in a clinicenvironment. Working under general supervision, this role promoteshealth and wellness for the congregations and community throughaccess to physical and spiritual care.

Nursing Assists community members in finding a primary careprovider in order to connect them with access to care.
Supports thehealth and wellness of members through personal health counselingto develop care plans.
Identifies health disparities, includingtransportation, housing, and food to remedy issues that arepreventing members from improving personal wellness.
Educatesmembers on disease prevention and management, medications, and thehealth system in order to increase knowledge and awareness.
Organizehealth screenings and fairs and flu clinics to support thereduction of illness in the community.
Completes screenings andassessments such as blood pressure and blood glucose monitoring toidentify potential health problems for treatment.
Provides spiritualcare through prayer and listening in order to support a holisticlifestyle.
Performs other duties as reasonably required or assigned.
EDUCATION
Required:
Bachelors Degree in Nursing
EXPERIENCE
Required:
5 years of clinical experience in a health caresetting
LICENSURE/CERTIFICATION/REGISTRY/LISTING
Required:
