This job is permanent, PRN.

• Performs initialand ongoing assessments of patient’s condition.
• Establishes aplan of care which is appropriate to problems identified andinvolves the patient according to his or her capabilities anddesires.
• Performs plan of care intervention, including medicationadministration, specimen collection, clinical treatments, as wellas other medical care treatment.
• Documents patient care given.
• Monitors, records, and communicates patient condition asappropriate to care team, physician, patient, and family.
• Assistspatients with performing activities of daily living, includingpersonal hygiene, elimination, nutrition, and ambulation.
• Collaborates as needed across disciplines to coordinate patientcare, including patient transfer, discharge, referral andspiritual/psychosocial support needs.
• Evaluates learning needs ofpatient and/or family and provides patient/family educationappropriate to age, culture, condition, and circumstances.
• Evaluates the effectiveness of patient care and identifies anyissues that affect desired patient outcomes.
• Performs rolespecific duties as assigned, including entering orders, performingadmissions and/or discharges, and coordinating transfers.
• Responds to complaints about patient care and related services, andmanages through established channels.
• Works as an advocate forthe physical and emotional well-being of the patient.
