• PRN for our GeroPsychDepartment.

This job is permanent, PRN.

• Performs initial and ongoing assessments ofpatient’s condition.
• Establishes a plan of care which isappropriate to problems identified and involves the patientaccording to his or her capabilities and desires.
• Performs planof care intervention, including medication administration, specimencollection, clinical treatments, as well as other medical caretreatment.
• Documents patient care given.
• Monitors, records, andcommunicates patient condition as appropriate to care team,physician, patient, and family.
• Assists patients with performingactivities of daily living, including personal hygiene,elimination, nutrition, and ambulation.
• Collaborates as neededacross disciplines to coordinate patient care, including patienttransfer, discharge, referral and spiritual/psychosocial supportneeds.

• Works as an advocate for thephysical and emotional well-being of the patient.
