Establishes a plan of care which isappropriate to problems identified and involves the patientaccording to his or her capabilities and desires. Monitors, records, andcommunicates patient condition as appropriate to care team,physician, patient, and family. Assists patients with performingactivities of daily living, including personal hygiene,elimination, nutrition, and ambulation. Collaborates as neededacross disciplines to coordinate patient care, including patienttransfer, discharge, referral and spiritual/psychosocial supportneeds. Works as an advocate for the physicaland emotional well-being of the patient.
