An active member of the interdisciplinaryhealthcare team the Case Manager facilitates internal and externaldelivery of healthcare services. The Case Manager integrates theroles of Utilization Management, Case Management and DischargePlanning. The Case Manager is responsible for care coordination anddevelopment of the discharge plan through collaboration with thehealthcare team and internal/external resources. The job objectiveis to promote a collaborative interdisciplinary care delivery modelto achieve optimal clinical and financial outcomes with adherenceto regulations and compliance.

Qualifications:

MinimumEducation Associate’s degree x Required Bachelor’s degree x Preferred Drag Edit Delete Required Skills

• Ability to understand, communicate, and react effectively topatient and family needs
• Critical thinking
• Work well independently while keeping calm and maintainingcustomer focus
• Must be able to work collaboratively with internal and externalresources for optimal discharge planning Drag Edit Delete Required Licenses Registered Nurse
• Current/active North Carolina or compact RN license
