Provides planning and delivery ofdirect and indirect patient care through the nursing process ofAssessment, Planning, Intervention, and Evaluation. Developsnursing care plans in coordination with patients, family andinterdisciplinary staff as necessary. Communicates changes inpatient’s clinical condition with Physicians, NursingSupervisor/Manager, and co-workers as appropriate. Participates indischarge planning process.
Essential Functions:
Maintains thestandard of nursing care and implements policies and procedures ofthe hospital and nursing department Directs, supervises, providesand evaluates nursing care provided to patients Assigns ordelegates tasks based on the needs and condition of the patient,potential for harm, complexity of the task, and within scope ofpractice of the staff to whom the task is delegated Assigns nursingcare team members in accordance to patient needs, team member’scapabilities and qualifications Documents patient admissionassessment and reassessments, patient care plans and otherpertinent information, completely in the patient’s medical recordaccording to nursing standards and policies Performs assessment onall patients on admission and reassessments as per policy. Makesreferrals to other disciplines based on assessment Develops nursingcare plan of assigned patient on admission, updates plan of care asneeded and ensures plan of care is coordinated with patient,family, and other members of the team Assesses and reassesses pain.
Utilizes appropriate pain management techniques. Educates thepatient and family regarding pain management Revises the plan ofcare as indicated by the patient’s response to treatment andevaluates overall plan daily for effectiveness Performs patientcare responsibilities considering needs specific to the standard ofcare for patient’s age Receives physician’s orders, ensurestranscription is accurate and documents completion. Administersmedication utilizing the five rights of medication administrationreducing the potential for medication errors Formulates a teachingplan based upon identified learning needs and evaluateseffectiveness of learning; family is included in teaching asappropriate Assists physicians with examinations, treatments andspecial procedures and performs services requiring technical andmanual skills within scope of practice. Performs treatments andprovides services to level of licensure Treats patients and theirfamilies with respect and dignity. Identifies and addressespsychosocial, cultural, ethnic, and religious/spiritual needs ofpatients and their families. Functions as liaison betweenadministration, patients, physicians, and other healthcareproviders Interacts professionally with patient/family and involvespatient/family in the formation of the plan of care Interprets dataabout the patient’s status to identify each patient’s age specificneeds and provide care needed by the patient group Performs allaspects of patient care in an environment that optimizes patientsafety and reduces the likelihood of medical/health care errorsInitiates or assists with emergency measures for sudden adversedevelopments in patients’ condition Answers telephone, pagingsystem, patients’ call lights, anticipates patients’ needs, andmakes rounds of assigned patients and responds as appropriateConsults other departments as appropriate to provide for aninterdisciplinary approach to the patient’s needs Provides end ofshift report to oncoming nurse, narcotics are counted,documentation is complete, and physician orders signed offCommunicates appropriately and clearly to management, co-workers,and physicians Identifies and addresses psychosocial needs ofpatients and family; communicates with Social Service/DischargePlanner regarding both in hospital and post discharge needsParticipates in orientation, instruction/training of new personnelManages and operates equipment safely and correctly
QualificationsEducation:
Graduation from an accredited Bachelor of Science inNursing, Associate Degree in Nursing or Nursing Diploma program
Licenses/Certification:
Current state licensure as Registered NurseBLS, NRP and Fetal monitoring certifications required De-escalationtraining required
Experience:
Minimum six months’ Obstetricalexperience in an acute care setting preferred
