Job Summary:
Deer Park Health and Rehab is seeking an experienced and detail-oriented MDS RN to manage and coordinate the assessment and care planning process for our residents. This position is responsible for ensuring accurate and timely completion of the Minimum Data Set (MDS) assessments and care plans in accordance with federal and state guidelines. The ideal candidate will possess strong clinical judgment, excellent documentation skills, and a commitment to quality resident care.
Key Responsibilities:
Complete and submit accurate MDS assessments, care plans, and related documentation in a timely manner. Coordinate interdisciplinary team participation in the assessment and care planning process. Review and interpret medical records, nursing notes, and resident assessments to ensure accuracy of data. Monitor and manage the RAI (Resident Assessment Instrument) process and ensure compliance with regulatory standards. Work closely with the reimbursement team to ensure proper Resource Utilization Group (RUG) classification and accurate Medicare/Medicaid billing. Identify and address variances or discrepancies in resident documentation. Serve as a clinical resource to staff for MDS-related questions and education. Participate in Quality Assurance and Performance Improvement (QAPI) initiatives as needed. Communicate effectively with staff, physicians, residents, and families to support care goals.
Qualifications:
Current and valid Registered Nurse (RN) license in the State of North Carolina. Minimum of 2 years of experience in long-term care nursing. At least 1 year of MDS experience preferred; AANAC certification a plus. Strong understanding of
MDS 3.0, RAI
process, and Medicare/Medicaid regulations. Exceptional attention to detail and organizational skills. Proficient in electronic health records (EHR) and MDS software systems. Excellent communication, analytical, and problem-solving skills.
