Required Qualifications:

• One year ofrecent experience as a Registered Nurse, including a minimum of sixmonths in a Home Health setting as an RN
• Proficient in OASISdocumentation
• Valid RN license in compliance with stateregulations
• BLS certification (AHA/ARC) Preferred Qualifications
• Home Health Nursing Certification (RN-BC)
• Other certificationsand licenses may be required for this position

Summary:

A HomeHealth Registered Nurse (RN) delivers direct nursing care topatients in their homes, ensuring comfort, stability, and effectivemanagement of their medical conditions. Home Health RNs assesspatient conditions, develop care plans, administer treatments, andeducate patients and caregivers on disease management.

EssentialWork Functions:

• Assess patient conditions and developindividualized care plans based on medical needs and physiciandirectives
• Administer medications, treatments, and wound carewhile monitoring patient responses
• Provide patient and caregivereducation on medication management, disease progression, andself-care techniques
• Monitor for changes in patient status,reporting concerns and adjusting care plans as needed
• Coordinatewith physicians, therapists, and other healthcare professionals toensure seamless care transitions
• Document patient care,interventions, and progress accurately and in compliance withregulatory standards
• Ensure compliance with infection control,safety, and home care best practices to protect patients andcaregivers
• Support families and caregivers, offering guidance andemotional support throughout the care process
• Maintaincommunication with case managers and interdisciplinary teams toprovide coordinated patient care
• Strong ability to completecharting and documentation requirements while meeting visit andproductivity goals
• Adhere to infection control, safety, andhealthcare regulations
• Perform other duties as assigned withinthe scope of practice

Required Essential Skills:

• The ability to read, write, & communicate in the Englishlanguage is required
• Ability to read/comprehend writteninstructions, follow verbal instructions, and proficiency in PCskills

• To work effectively with a variety ofpersonnel (professional and ancillary) to present a positiveattitude and professionalism

• Ability tograsp, fine manipulation, pushing/pulling and able to move aboutwhen assisting with procedures and/or using department equipment.

• Must be able to cope with frequent contactwith the general public, customers, and meeting deadlines underpressure. Must be able to work under close supervision occasionallyas well as working without assistance from other personnel. Must beable to contend with irregular activity schedules occasionally andcontinuous concentration to detail
