We are seeking an experienced and dedicated Registered Nurse (RN) to join our team on a contract basis for a client in Charlotte. Key Responsibilities:
Provide comprehensive nursing care to patients in their homes. Perform initial and ongoing patient assessments, including vital signs, medication management, and wound care. Develop and implement individualized care plans based on patient needs. Educate patients and their families on disease management, treatment plans, and preventive care. Collaborate with physicians, therapists, and other healthcare professionals to ensure a cohesive care plan. Document all patient care activities accurately and promptly in electronic medical records (EMR). Monitor and report changes in patient condition to the supervising physician. Ensure compliance with all state and federal regulations related to home health care.
Qualifications:
Active RN license Minimum of 3 months of home health experience using
OASIS HCHB
experience is a big plus Strong clinical skills, including IV therapy, wound care, and medication management. Proficient in using electronic medical records (EMR) systems. Excellent communication and interpersonal skills. Ability to work independently and manage time effectively. Valid drivers license and reliable transportation.
