Duties:
- Collects and analyzes data pertinent to thehealthcare consumer’s health or situation.
- Identifies expectedoutcomes and develops individualized plans to attain measurableoutcomes.
- Implements the identified plan, coordinates caredelivery, and promotes health and a safe environment.
- Delegateselements of care appropriately and evaluates progress toward goalsand outcomes.
