Full Job Description Clinical Nurse Liaison Schedule:

A rotating 4 ten hour shifts to include holidays and weekends. 7 AM to 5 PM What you’ll be doing: As a registered nurse in the Clinical Nurse Liaison role, you will provide quality care to patients from diverse socioeconomic and cultural backgrounds. You’ll be working within an acute care setting that is often fast paced and you’ll need to be able to pivot quickly with the changing environment with complex patients seen in a Level II Trauma center. You’ll be developing relationships with referral sources like specific providers, case managers, social workers and staff nurses within the hospital system to enhance the admission process and provide community support. To do so you’ll need to be: Familiar with marketing and strategic goals for the company. Be confident and knowledgeable about the services offered by Lower Cape Fear LifeCare. Excellent customer service skills to ensure satisfaction. Able to manage multiple tasks simultaneously and quickly pivot from one task to another as needed. Understand hospice eligibility and advise on the most appropriate care settings. Being comfortable providing one on one educational in-services with medical staff on topics such as: Hospice 101 – Eligibility and Philosophy End-of-Life and Symptom Management Specific Disease States (e.g., dementia, heart failure, COPD, end-stage renal disease) Medication usage in Hospice While you will not be providing direct patient care as a registered nurse, you’ll spend time working as a consultant with patients and their families providing education on identifying signs and symptoms, common medications used in hospice care, and what to expect at end-of-life. You’ll work to collaborate and coordinate with external medical staff and referral sources to determine patient eligibility for hospice, palliative care, or our Memory Partners program. If patients meet admissions criteria, you’ll work to coordinate transition of care into one of our three inpatient hospice care centers, in-hospital hospice, outpatient case managers to receive hospice care at home or hospice services in a facility setting. This transition of care would include things like: Obtaining consent to treat from the patient or primary caregiver. Working with physicians and case managers on discharge planning. Coordinating with the hospital you are based at to organize patient pick up and arrival times. Completing documentation for new referrals and follow-ups after each visit. Working with LCFL staff to start the admissions process and order DME and other supplies. Document in both the hospital EMR and Lower Cape Fear LifeCare EMR systems What we’re looking for:

Education:

Graduate of an accredited school of nursing, either through an AD, Diploma or BSN program. Degree in business, marketing, or related field preferred. Licensure /

Certifications:

Current license to practice professional nursing in the State of North Carolina or South Carolina depending on position location.

Experience:

Two years experience. Prefer hospice or home care. Prefer experience in marketing, public affairs, community relations or hospital liaison nurse.
