JOB SUMMARY

Maintain knowledge of state licensure, federal certification, and accreditation requirements. Provide coordination, case management of all patient services. Complete phone assessment of patient needs and anticipate appropriate services for patients. Organizes all patient communication, patient needs and conversion to Liberty Program in Excel and or Axxess.

JOB REQUIREMENTS

Current RN or LPN licensure in state of practice. Successful completion of a nursing education from an approved school of nursing. Two years of clinical nursing experience and at least one year of hospice nursing experience preferred. Must have knowledge of the framework, function, and organization of all Liberty Continuums to include hospice, homecare and the palliative program. Requires in-depth knowledge of hospice/homecare Medicare regulations. Must maintain a working knowledge of the principles and practice of nursing according to nursing practice act, agencies policies and procedures, and reimbursement guidelines. Must be able to independently organize work. Must be able to communicate effectively, both orally and in writing. Constantly able to use a computer and have a working knowledge of a variety of computer applications. Must have a valid drivers’ license.
