JOB SUMMARY
Maintain knowledge of state licensure, federal certification, and accreditation requirements.
Provide coordination, case management of all patient services.
Complete phone assessment of patient needs and anticipate appropriate services for patients.
Organizes all patientmunication, patient needs and conversion to Liberty Program in Excel and or Axxess.
JOB REQUIREMENTS
Current RN or LPN licensure in state of practice.
Successfulpletion of a nursing education from an approved school of nursing.
Two years of clinical nursing experience and at least one year of hospice nursing experience preferred.
Must have knowledge of the framework, function, andanization of all Liberty Continuums to include hospice, homecare and the palliative program.
Requires in-depth knowledge of hospice/homecare Medicare regulations.
Must maintain a working knowledge of the principles and practice of nursing according to nursing practice act, agencies policies and procedures, and reimbursement guidelines.
Must be able to independentlyanize work. Must be able tomunicate effectively, both orally and in writing.
Constantly able to use aputer and have a working knowledge of a variety ofputer applications. Must have a valid drivers’ license.
Minimum of twelve (12) hours continuing education required annually.
