Beds Open Position/Department:

Hospital-based Care Manager Women & Children’s Service Line (PREFERS candidates w/ pediatric nursing experience) 20-25 cases at 1 time

Role:

will play a crucial role in providing comprehensive and coordinated care to patients within UNC Health. This position involves utilizing a variety of skills, including risk segmentation, patient assessments, patient-centered care plans, tasks or interventions, care transitions, delegated work, and payer communication. The Care Manager works collaboratively with healthcare providers, patients, and their families to ensure that patients receive the highest quality of care and support throughout their healthcare journey.

Pay Details:

$34.65, starting pay and goes up based on experience (Exempt/Salary) LPN/CNA experience considered Pay differential for 2, 3, and weekend shifts

Shifts/Hours Open:

Full-time hours 8:30a-5p | 40 hrs/wk (flexible scheduling/start times may be available) E/O weekend required

Qualifications:

North Carolina RN License•Compact or NC State BLS ASN, agreement to enroll in BSN if not already enrolled or obtained 2-3 years of acute care/patient-population, case specific/bedside care experience
