Job Description:

We areseeking a skilled and compassionate Home Health and Hospice NurseEducator to join our team in a hybrid role. This position is idealfor a Registered Nurse with strong OASIS assessment and home healthexperience, who is passionate about teaching, supporting patientcare teams, and providing hands-on training and assistance asneeded. The Nurse Educator will be responsible for educating staff,assisting with patient escalations, and providing in-home trainingand support for care teams. This role requires flexibility and awillingness to go wherever the need arises, including traveling topatient homes when necessary.•

Key Responsibilities:
Staff Education & Training:

Educate and mentor physicians,staff, and home health professionals to ensure the delivery ofhigh-quality, compassionate care. Provide hands-on training andsupport for clinical staff in both office and home settings.

OASIS Assessment:

Conduct OASIS assessments and ensurecompliance with regulatory requirements. Train staff in completingthorough, accurate OASIS documentation.

Patient Support & Escalations:

Provide on-site assistance inpatient homes when needed, especially during patient escalations,to ensure appropriate interventions are in place.

Regulatory Compliance:

Educate and ensure adherence to stateand federal regulations related to home health and hospice care.

Act as a resource for staff and referral sources regardingregulatory requirements and best practices.

Flexible Role:

Willingness to travel to Raleigh and Chapel hilloffice locaitons, patient homes as needed to train, assist withduties, and support care team efforts.

Required Qualifications:
Education:

Graduation from a state-accredited school ofprofessional nursing (BSN preferred).

Licensure:

Must be a licensed Registered Nurse (RN) in thestate of North Carolina.

Experience:

A minimum of 2 years of home health, hospice, orpublic health experience. Strong background in OASIS assessments isrequired.

Skills:

Excellent communication, teaching, and clinical skills.

Ability to work independently and as part of a team. Strongorganizational and problem-solving abilities.

Work Schedule:

Standard 40 hours per week, typically 5-8 hour shifts.

Consideration will be given for 4-10 hour shifts.

based on experience and internal equity.
