Required Qualifications:
One year of recent experience as an RN Case Manager Valid RN license in compliance with state regulations Current BLS (AHA/ ARC)
Certification Preferred Qualifications:
ACLS Certifications (AHA/ARC) Other certifications and licenses may be required for thisposition
Summary:
The Case Manager RN coordinates and facilitatespatient care across the healthcare continuum, ensuring optimal useof resources and improved patient outcomes. They assess patientneeds, develop comprehensive care plans, and collaborate withmultidisciplinary teams to provide efficient, cost-effective, andhigh-quality patient care.
Essential Work Functions:
Assess patients’ medical, psychosocial, and functionalneeds Collaborate with interdisciplinary teams, including physicians,therapists, social workers, and case managers, to developpatient-centered care plans Develop and update individualized care plans, aligning withpatient goals and clinical guidelines Coordinate transitions of care ensuring seamless services andresources across the continuum of care Facilitate communication between patients, families, andinterdisciplinary team members Monitor and evaluate patient outcomes, and adjust care plans asneeded Ensure compliance with regulatory standards and insurancerequirements Advocate for patients’ preferences addressing barriers to careand promoting equitable access to services Conduct utilization reviews and manage length of stay ensuringadherence to evidence-based guidelines for care delivery Educate patients and families on health conditions andavailable community resources Document case management comprehensively and accurately in thepatient’s medical record Participates in interdisciplinary team meetings Perform other duties as assigned within scope Adhere to hospital safety protocols, infection controlguidelines, and regulatory standards
Required Essential Skills:
Critical thinking, service excellence and good interpersonalcommunication skills The ability to read, write, & communicate in the Englishlanguage is required Ability to read/comprehend written instructions, follow verbalinstructions, and proficiency in PC skills Physical Abilities
- Remain in a stationary position, moveabout, move equipment (50-100lbs), pushing, pulling, bending Interpersonal Skills
- To work effectively with a variety ofpersonnel (professional and ancillary) to present a positiveattitude and professionalism Technical/Motor Skills
- Ability to grasp, fine manipulation,pushing/pulling and able to move about when assisting withprocedures and/or using department equipment. Mental Requirements
