This office services our adult and geriatric clients on aper visit basis throughout Caswell, Granville, Nash, Person, Vance,and Warren counties . Responsibilities:

Followa designated care plan in accordance with patient’s needs Make homevisits to clients in designated geographic territories Performassigned duties, including administration of medication, woundcare, treatments, and procedures Monitor clients’ conditions;reporting changes to Clinical or Client Services Manager Follow upwith, execute, and properly document doctors’ orders Perform clientassessments as necessary Case management and coordinationAccurately document observations, interventions, and evaluationspertaining to client care management and services provided,utilizing a state-of-the-art touch pad tablet Qualifications for aRegistered Nurse (RN): A current license as a Registered Nurse inNorth Carolina. A minimum of one year of recent, verifiableclinical (medical/surgical) experience. Prior home care experiencestrongly preferred, but not required. Graduation from an accreditedand approved nursing program, as indicated by school transcript ordiploma. Solid computer skills; prior experience with electronicmedical records (EMR) preferred.
