  • Provide home-basedcare to patients with illnesses, injuries and chronicdiseases—often after discharge from a hospital, rehab or nursingfacility.
  • Work with a team of physicians and oversee amultidisciplinary team providing patient care.
  • Examine patients,do assessments, document progress, and report changes to theirphysician.
  • Educate patients on their plan of care, prescribedmedication, therapy, diet and exercise.
  • Coach family members onthe patient’s plan of care, medication, and home safety.
  • Ensuregoals are met and coordinate discharge from services. RequiredSkillsA few must-haves for Home Health Registered Nurses (RN):
  • Graduate of an accredited Registered Nurse (RN) program
  • Active,unrestricted Registered Nurse (RN) license in the State of SouthCarolina or compact multi-state license
