Collaborates with local pediatrician offices, WIC, CDSA, Randolph Hospital, etc. to increase community awareness of the Case management for children birth up to age five program and to receive referrals for care management services. The Case Management for children birth up to age five program provides formal case management services to the target population who meet program guidelines. All children are eligible including those who have Medicaid, Insurance or no Insurance coverage. The process includes; outreach, population identification, assessment, risk stratification, interventions, integration with provider, collaboration with the network, training and staffing. Helps ensure all children birth to age 5 who qualify for Case Management for children birth to age five program that services are followed according to State Contract and DMA guidelines.

The Case Management for high risk pregnant women program provides formal case management services to pregnant. Medicaid women who are determined to be at high risk for poor birth outcomes. The process includes; outreach, population identification, assessment, risk stratification, interventions, integration with provider, collaboration with the network, training and staffing. Helps ensure all high risk pregnant Medicaid women are followed according to State Contract and DMA guidelines.

EXAMPLE OF DUTIES

Graduation from a four-year college or university with a B.S. in Nursing which includes a Public Health Nursing rotation and one year of Public Health Nursing experience; or Master’s in Public Health and graduation from a school of professional nursing and one year of professional nursing experience; or graduation from a school of professional nursing and two years of professional nursing experience including one year of Public Health Nursing experience; or an equivalent combination of training and experience.
