Job Summary:
Responsible for providingskilled nursing care to residents according to established careplan and in compliance with federal and state regulations. Providesguidance and direction to ancillary staff.
Job Responsibilities andRequirements:
PRIMARY RESPONSIBILITIES
Provides quality care toacute, subacute, and chronically ill residents, administersmedications and treatments according to standards of care, andmaintains competency in all required nursing skills. Providesdirection to CNAs and monitors the workflow of the unit to ensurecoordination of care.
Uses nursing process for:
assessment, nursingdiagnosis, planning, intervention, and evaluation. Reportsignificant changes to physician as indicated. Maintains accurateinventory of medications at the beginning and end of each shift.
Ensures accurate distribution of medications to all residentsaccording to the 5 rights. Monitors for medication side effects.
Ensures that medications are provided in a manner consistent withmanufacturer’s guidelines. Gives guidance and direction toauxiliary staff. Participates in report meetings at the beginningand end of each shift. Communicates with residents, guardians, andfamily members on the current condition and needs of residents inaccordance with regulations and standards of practice. Providesteaching and training to residents and families as needed. Ensuresthat preventative measures are in place such as: specialtymattresses, foam boots, weekly skin checks, turning/repositioningschedules, toileting schedules, and cushions in all wheelchairs.
Follows up on reports of rashes, open areas, edema, blisters, skintears, or bleeding. Responds promptly to alarms. Ensures thatrestraint alternatives are in place/activated including: personalalarms, cushion alarms, wheelchair belts, tumble mats, alarm mats,bolstered beds, hip protectors. Provides and ensure quality end oflife care. Uses comfort basket to meet resident and family needs.
Ensures hygiene, comfort, and spiritual needs are met. Treatspain/restlessness. Offers family courtesy meal trays and otheramenities. Demonstrates compliance with post-mortem careprocedures. Participates in departure ceremony. Works in a constantstate of alertness and safe manner. May perform point of caretesting according to policies and procedures. Performs other dutiesas assigned.
EDUCATION
Graduate of accredited school of nursing oreducation equivalency for licensing

PHYSICAL REQUIREMENTS
Constant use of speech to shareinformation through oral communication. Constant standing andwalking. Frequent lifting/carrying and pushing/pulling objectsweighing 0-25 lbs. Frequent sitting, reaching and keyboard use/dataentry. Frequent use of vision and depth perception for distancesnear (20 inches or less) and far (20 feet or more) and to identifyand distinguish colors. Frequent use of smell to detect/recognizeodors. Frequent use of hearing to receive oral communication,distinguish body sounds and/or hear alarms, malfunctioningmachinery, etc. Occasional lifting/carrying and pushing/pullingobjects weighing 25-50 lbs. Occasional lifting/moving of patients.
Occasional bending, stooping, kneeling, squatting, twisting,gripping and repetitive foot/leg and hand/arm movements. Occasionaldriving. Rare crawling and running.
