Responsibilities:

The CareManagement Nurse, MDS Nurse works the RAI process and conductsassessments and care plan coordination for those residentsassigned. Some responsibilities of the MDS nurse include: Completesthe MDS, CAA’s and care plans within regulated time frames.

Assesses resident through physical assessment, interview and chartreview. Discusses resident care needs with care givers, includingphysician, nursing, social services, therapy, dietary, and activitystaff. Reviews information from hospital, consults and outsideagencies and uses such information in the completion of theassessment and care planning. Coordinates, identifies, and/orinitiates significant change MDS’ Is prepared to conduct PPSmeetings maintaining MDS assessments per Medicare schedule andmaintains PPS board for monitoring of Medicare days and RUGsutilization in the absence of the Care Management CoordinatorRemains current with the American Association of Nursing AssessmentCoordinators (AANAC) requirements.

Qualifications:

RegisteredNurse, RN or Licensed Practical Nurse AANC certification a plus.

RAC-CT Knowledge of the Resident Assessment Instrument (RAI)process, including the principles the Prospective Payment Process(PPS) strongly preferred.
