Performs all activities in accordance with approved policies andguides. Assessments include physical, psychosocial, environmental,self-care abilities, educational, age appropriate, cultural anddischarge planning needs. Performs plan of care intervention, including medicationadministration, specimen collection, clinical treatments, as wellas other medical care treatment. Documents patient care given. Monitors, records, and communicates patient condition asappropriate to care team, physician, patient, and family. Assists patients with performing activities of daily living,including personal hygiene, elimination, nutrition, andambulation. Collaborates as needed across disciplines to coordinate patientcare, including patient transfer, discharge, referral andspiritual/psychosocial support needs. Works as an advocate for the physical and emotional well-being ofthe patient. Assists with orientation and training of others as directed. Minimum Education Associate’s Degree in Nursing required Bachelor’sDegree in Nursing preferred Required Skills Requires critical thinking skills, decisivejudgment and the ability to work with minimal supervision. BasicLife Support (BLS) required Handle with care within 4 months ofhire Minimum Work Experience 1-3 years preferred and recent acute careexperience within past 3 years preferred.
