The RN Case Manager plans, organizes, drives case management development, directs Home Care Services/Catered Living Program and is experienced in geriatric nursing, with emphasis on community health education/experience. The RN Case Manager builds from the resources of the Home Care Department and community, to plan and direct services to meet the needs of individuals and families within their homes. QUALIFICATIONS

Graduate of an accredited school of nursing. One (1) to two (two) years of recent acute care/geriatric experience in an institutional setting. Current licensure in state, CPR certification and valid driver’s license. Associates Degree in Nursing (ADN) or Bachelors of Science Degree in Nursing (BSN), with one (1) year of Home Health Care experience preferred. Supervisory experienced desired. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Marketing/Case Management experience preferred. Prolonged or considerable walking or standing. Able to lift, position or transfer clients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills. If you are required to drive a golf cart you must have a valid and active driver’s license issued from any state. You must successfully complete the Cypress of Raleigh golf cart training. If you are using one of the Cypress vehicles to drive a member, you must have a good driving record and active driver license from any state. Must be flexible, able to work evenings and weekends.
