RN – Transitional Coord Position Available In Wilson, North Carolina

As a member of the interdisciplinary healthcare team this position works independently assessing post discharge needs. Goals for TCS is to educate patients/families on their disease process and coordination of care post discharge identifying resources that may be needed as well as ensuring appropriate resources are present for high risk patient population. TC RN promotes empowerment of the patient in caring for themselves.
