HOSPICEREGISTERED NURSE

(RN) Per Diem (Yadkin County)

Job Description:

Apply a working knowledge of nursing theories and concepts.

Responsible for care and documentation of care which meetsreimbursement guidelines. Coordinate physical care of the patientwith family, patient, physician and Hospice team. Communicate withpatients/families at all levels. Communicate with all agencypersonnel. Attend and participate in IDT meetings and qualityreview as scheduled by agency.

Job Requirements:

Current RNlicensure in state of practice, successful completion of a nursingeducation from an approved school of nursing with two years ofclinical nursing experience and at least one year of hospicenursing experience preferred. Knowledge of the framework, function,and organization of a home health and hospice care program and thenurse’s role as a member of the patient care team. Workingknowledge of the principles and practice of nursing according tonursing practice act, agencies policies and procedures, andreimbursement guidelines. 12 hours of continuing educationannually.
