Job Summary:
/n An Acute Rehabilitation / Transitional CareUnit (TCU) Registered Nurse (Acute Rehab / TCU RN) providesspecialized nursing care to patients recovering from acuteillnesses, surgeries, or injuries in an intensive rehabilitationsetting. The Acute Rehab / TCU RN focuses on managing complex careneeds, promoting recovery, and facilitating the transition ofpatients from acute care to home or lower levels of care, ensuringoptimal outcomes. /n/n
Key Responsibilities:
/n /n
- Assess, plan,and provide nursing care to patients in acute rehabilitation andtransitional care units recovering from illnesses, surgeries, orinjuries. /n
- Collaborate with multidisciplinary teams, includingtherapists, social workers, case managers, and physicians, todevelop and implement individualized care plans. /n
- Monitor andmanage patients’ vital signs, pain levels, mobility, and progresstoward rehabilitation goals. /n
- Administer medications, IVtherapies, wound care, and other treatments related to the recoveryprocess. /n
- Provide education and support to patients andfamilies regarding care plans, medication management, wound care,physical therapy, and self-care strategies. /n
- Assist patientswith daily activities such as mobility, transfers, and personalcare, promoting independence and safety. /n
- Recognize and respondto potential complications such as infections, pain, or mobilityissues, implementing interventions as needed. /n
- Document patientassessments, interventions, and outcomes accurately and incompliance with hospital policies and regulatory standards. /n
- Facilitate patient transitions from acute care settings to home oroutpatient services, collaborating with case managers, socialworkers, and other team members. /n
- Participate ininterdisciplinary team meetings, contributing input on patientprogress, goals, and discharge planning.

Qualifications:

Education:
Bachelor of Science in Nursing (BSN) preferred;Associate Degree in Nursing (ADN) accepted. /n

Licensure:
/n /n
- Valid Registered Nurse (RN) license in the state of practice. /n /n/n

Certification:
/n /n
- Basic Life Support (BLS) certificationrequired. /n
- Advanced Cardiac Life Support (ACLS) and/orCertified Rehabilitation Registered Nurse (CRRN) preferred.
