Care Management Nurse (Home & Hospice) 5.0 5.0 out of 5 stars Littleton, NH 03561 A leading provider of Home Health and Hospice services in New Hampshire is looking to add a Registered Nurse Case Manage r to its compassionate care team, serving the Littleton and Whitefield region. In this role, you’ll provide comprehensive, high-level care in patients’ homes, delivering direct support, managing care plans, and coordinating treatment across disciplines.
Responsibilities:
Offer direct, hands-on nursing care to medically complex patients, including pain management, wound vac therapy, and IV infusions Perform thorough assessments at admission and throughout treatment Create and manage individualized care plans in collaboration with patients, families, and providers Maintain open communication with physicians, specialists, and community partners Deliver ongoing education to patients and their caregivers to support health and recovery at home Administer prescribed medications as needed
Qualifications:
ADN required, BSN preferred Active NH RN license. 2+ years home health experience strongly preferred.
