Job:

The Case Manager RN will awork primarily in the EmergencyDepartment, and collaborate with other members of the health careteam to identify appropriate utilization of resources and a safeand effective transition plan ensuring clinical and socialdeterminants of health are met. With the patient, family/caregiver,and health care team, create a transition plan appropriate to thepatient’s needs and resources including community providers toensure effective communication and collaboration with a successfultransition plan.

Schedule:

You will work 36 hours per week, (3 12-hour shifts), and everyother Saturday.

What You’ll Do:

Assesses and identifies patients’ needs during the dischargeplanning process Provide resources and community referrals for continuity ofcare Care coordination with families, providers, and clinical team Follows Medicare conditions when needed

Utilization Reviews Who You Are:

Bachelor’s degree in a healthcare-related field or equivalentexperience – Current BSN or commitment to achieving the degree in aspecified amount of time 3 – 5 years of case management-related health care experience. CCM or ACM preferred Excellent clinical documentation, and successful usage of EMRsystems preferred
