Job Description:

We are seeking a Nurse Practitioner (PRN) for home riskassessments, welcoming new grads. Conduct up to 1-hour visits evaluating health, medical history,medications, social factors, and risks to improve care and identifyundiagnosed conditions. Collaborate with healthcare teams, provide patient education, anddocument assessments per guidelines. Visit patients in homes, skilled facilities, or via telehealth inMount Sterling, KY, with flexible scheduling including days,evenings, and weekends. Earn $1,800

• $9,000/month based on assessment volume. Require Master
• s degree or equivalent, current RN/NPlicensure, certification, and at least 1 year NP experience. Must have travel ability, valid driver
• s license, and autoinsurance.
