Location :
Merrimack, NH Shifts:
3 shifts 7am to 7pm for 36 hrs. to 40 hrs. a week

Responsibilities Accessing, collecting data, and evaluating the patient’s physical, spiritual, and emotional needs. Provides preventative and therapeutic treatment under medical direction. Creates a nursing plan of care with input from the patient, family, and provider to include measurable goals and behavioral objectives. Qualifications Graduate of an accredited school of nursing. Associate’s degree in nursing. Strong skills in symptom management. One year recent acute care experience. Experience managing oncology patients and those with co-morbidities. Preferred Qualifications Bachelor’s degree in nursing. Community Health/Education experience. Certification in hospice and palliative care a plus. Knowledge or experience with end-of-life care and hospice care is beneficial. EMR software proficiency. We use EPIC. Required Registered as a RN with New Hampshire’s Board of Nursing. CPR Certification. Satisfactory criminal record. Position requires sharing in every other weekend rotation and agency holidays. We value diversity — in backgrounds and in experiences.
