Part-Time Saturday and Sunday, 9:00 am – 1:00 pm with on call coverage. Come join a great team at our Hospice House and spend your work days doing meaningful work. Granite VNA is looking for a Hospice Nurse Practitioner whose passion is providing care to those on their end of life journey. In this role, the APRN works with the Hospice Medical Director to gather, document, and share clinical findings in order to determine continued hospice eligibility. The Hospice APRN will perform the face to face encounters for hospice patients in accordance with the patient’s plan of care, change in clinical status, and regulatory requirements 30 days prior to the start of the 3rd benefit period (180 days re-certification) and then again every 60 days thereafter. Essential Responsibilities Oversees the medical care of hospice patients in collaboration with the Medical Director and attending provider. Provide palliative care consultation for hospice patients as needed. Communicates with attending physicians as need regarding their patient’s condition and care. Assumes responsibility for the medical care of Hospice patients if the attending is unable to provide needed medical care. Works with Hospice Medical Director and the Hospice Interdisciplinary Team (IDT) to ensure the provision of physical, emotional, spiritual, social and bereavement care to adult, geriatric, pediatric, and neonatal Hospice Patients and their families. Assists in determining the adult, geriatric, and pediatric patients’ medical eligibility for hospice services by visiting the patient and performing a face to face encounter in accordance with the patient’s plan of care, change in clinical status and regulatory requirements. Completes an attestation in writing, documenting that he or she had the face to face encounter, the date of the visit, the clinical findings supporting the ongoing eligibility for Hospice services, and the fact these clinical findings were provided to the certifying physician. Serves as a liaison between the attending physicians, staff, adult, geriatric, and pediatric patient and families as requested regarding pain and other symptom management to assure the patient/family needs are met. interviews patients and/or families to obtain comprehensive history and data about patient’s physical and developmental status and behavior. Examines the patient to obtain objective physical data. Completes a physical and psychological assessment based on interview, examination and available data. Identifies medical and health issues and needs based upon evaluation. Formulates differential diagnosis based on history, physical and clinical findings. Develops a treatment plan which may include diagnostic test, pharmaceutical and non-pharmaceutical interventions as well as education. Reviews clinical data from lab results, x-rays, scans and pathology reports. Performs neurological, cardiac, respiratory and other testing indicated by the evaluation. Administers medication, injections, dressing changes and other nursing interventions and duties. Collaborates with the interdisciplinary team in establishing, implementing and reviewing the Hospice plan of care for each patient in keeping with the Conditions of Participation (COPs). Other Job Responsibilities Attends patient care conferences and hospice team meetings on a routine basis to provide consultation and direction. Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services rendered. Collaborates with other Agency program staff to provide consultation and evaluation to patients in home, outpatient, facility and inpatient settings, as scheduled and as needed. Prepares documentation needed for reimbursement for services rendered. Meets agency productivity expectation. Assures continuing education requirements of the NH Nurse Practice Act are met. Assures the practice is within the rules/regulations of the NH Nurse Practice Act. Maintains good communication and positive work relationships with the Hospice Team, internal and external parties including, but not limited to, area physicians, nursing home staff and referral sources. Provides ongoing community and professional education as it relates to medical community and external professional audiences, as requested. Demonstrate flexibility in role by accepting other duties as assigned for which qualified.

Requirements:

Registered Nurse with an advanced degree in nursing and/or health care administration who: Holds a current New Hampshire NP/APRN license 3-4 years recent ARNP experience in hospice or palliative care setting Experience in the area of pain and symptom management Certification in Hospice and Palliative Care Preferred.
