In this role, you will provide high-quality, patient-centered hospice care, ensuring comfort, dignity, and support for patients and their families. As a key member of the interdisciplinary team, you will coordinate and oversee individualized care plans, symptom management, and end-of-life support in home or facility settings.

This role requires a Hospice RN Case Manager to travel to our service areas of Burlington, Camden & Gloucester counties .

Job Responsibilities:

Provide skilled nursing care to hospice patients in accordance with their care plans. Assess patient conditions, manage symptoms, and adjust care plans as needed. Educate and support patients, families, and caregivers on end-of-life care and comfort measures. Coordinate with physicians, social workers, chaplains, and other team members to ensure holistic patient care. Document patient status, treatments, and interventions in compliance with hospice regulations. Participate in interdisciplinary team meetings and contribute to care planning. Maintain compliance with Medicare, Medicaid, and state/federal hospice regulations. Participate in on-call rotations as needed.

Qualifications/Skills:

Education:

Associate or Bachelor’s Degree in Nursing (ASN/BSN) from an accredited program.

Licensure:

Active and unrestricted Registered Nurse (RN) license in New Jersey.

Experience:

Minimum of two year of nursing experience , preferably in hospice, palliative care, or home health.

Certifications:

CHPN (Certified Hospice and Palliative Nurse) preferred but not required.

Skills:

Strong clinical assessment, communication, and organizational skills.

Other:

Work Schedule :
