Position Summary:
The Infusion RN is responsiblefor providing specialized nursing services to patients andcaregivers at home or in alternative settings. Provides technicalnursing and skilled interventions as ordered by the physician.
Instructs motivates, and assists patients / caregivers in learningand improving ability to live independently at home.
EssentialFunctions and Job Responsibilities:
- Makes initial visits,processes orders, notifies physician of patient needs and changesin condition.
- Initiates care completescertification/recertification orders, and discharge summaries, ifrequired.
- Regularly re-evaluates needs of the patients.
- Counsels with the patient and family/significant others on nursing,teaching, and related needs.
- Inserts intravenous cannulas;teaches and or administers prescribed intravenous solutions,medications, and biological products; monitors and maintainsinfusion sites and systems; evaluates response to prescribedtherapy.
- Informs the physician and personnel of changes in thecondition and needs of the patient.
- Initiates appropriatepreventative and rehabilitative nursing procedures.
- Providesthose services requiring substantial specialized nursing skills.
- Prepares clinical records, care plans, progress notes for eachpatient visit and summaries of care conferences on his/her patientsin a timely manner per company policy.
- Provides case managementalong with the pharmacist to enhance patient outcomes.
- Identifiesnursing diagnosis/patient care problems and interventions makingadditions/changes to ensure the patient’s problems arecomprehensively addressed to avoid rehospitalization.
- Initiatesand or coordinates the plan of care to include other disciplines asneeded and act as a case manager to ensure integration of care isprovided.
- Assesses the patient’s environment with particularattention to safety to ensure the environment is suitable for thedelivery of safe and effective care.
- Participates in in-servicesprograms and presents in-service programs.
- Conducts patient careconferences on patients assigned to his/her care.
- Attends allpatient care conferences and meetings as scheduled.
- Participatesin peer review and performance improvement as assigned.
- Participates in review of clinical records as assigned.
- Assumeon-call responsibilities during non-business hours in accordancewith company policy.
- Maintain patient confidentiality andfunction within the guidelines of HIPAA.
- Complete assignedcompliance training and other educational programs as required.
- Maintain compliant with AdaptHealth’s Compliance Program.
- Performother related duties as assigned.

- Administer infusion nursing care; instruct, motivate,and assists patients and caregivers in managing their healthstatus.
- Strong skills in placing PIVs, and managing Ports, andPICCs.
- Knowledge and ability and the necessary skill andjudgement to perform all procedures as outlined.

- Knowledge of the regulatoryrequirements at the state, federal and local level

- Computer skillsincluding knowledge of Microsoft Office applications.
Requirements:
Education and Experience Requirements:
- Graduate of an accreditedSchool of Nursing.
- Two (2) years nursing experience in a hospitalor similar setting required.
- Two (2) years of infusion therapyexperience. Preferred
- Must have a valid and unrestricted driver’slicense.
- Must have a valid and unrestricted RN license from thestate in which he/she would be practicing.
- CRNI credentialpreferred.
- Current CPR certification
Physical Demands and WorkEnvironment:
- Must be able to bend, stoop, stretch, stand, and sitfor extended periods.
- Must be able to perform one-man CPR.
- Ability to perform repetitive motions of wrists, hands and/orfingers due to extensive computer use.
- Work environment may bestressful at times, as overall office activities and work levelsfluctuate.
- Subject to long periods of sitting and exposure tocomputer screen.
- May be exposed to hazardous materials, loudnoise, extreme heat/cold, direct, or indirect contact withairborne, bloodborne, and/or other potentially infectious pathogen
- May be exposed to angry or irate customers, patients, or referralsources.
- Ability to utilize a personal computer and other officeequipment.
- Assist in pulling a patient with a weight ofapproximately 130 pounds to a sitting position.
- Must be able tolift 45 pounds as needed.
- Physical and mental ability to provideclinical assessments.
- Requires travel throughout service area.
- Ability to effectively communicate both verbally and written withinternal and external customers with the ability to demonstrateempathy, compassion, courtesy, and respect for privacy.
- Must beable to access the patient’s residence.
