Primary Care Pediatric Nurse Practitioner pediatricsa1 Jersey City, NJ 07306 Job Overview We are seeking a dedicated and skilled Primary Care Nurse Practitioner to join our healthcare team. In this role, you will provide comprehensive care to patients in an outpatient setting, focusing on preventive health, chronic disease management, and overall wellness. You will work collaboratively with a multidisciplinary team to deliver high-quality patient care and ensure optimal health outcomes. Duties Conduct thorough patient assessments, including taking vital signs and obtaining medical histories. Develop and implement individualized care plans based on patient needs and preferences. Provide case management services to coordinate patient care across various healthcare settings. Manage infectious disease care, ensuring appropriate treatment and follow-up for affected patients. Utilize systems such as Athenahealth for electronic health record management and documentation. Perform sonography procedures as needed to assist in diagnosis and treatment planning. Educate patients on health maintenance, disease prevention, and self-management strategies. Collaborate with physicians and other healthcare professionals to optimize patient outcomes. Stay updated on best practices in outpatient care and participate in ongoing professional development. Requirements Current certification as a Nurse Practitioner with a focus on primary care. Proven experience in patient assessment and management in an outpatient setting. Familiarity with coding systems relevant to healthcare documentation and billing processes. Strong skills in case management, particularly for chronic diseases and infectious conditions. Proficiency in using electronic health record systems, especially Athenahealth. Ability to perform vital sign assessments accurately and efficiently. Knowledge of sonography techniques is preferred but not mandatory.
