The above reflects the anticipated salary range for this position if hired to work, The Compensation offered to the candidate selected for the position will depend on several factors, including the candidate’s educational background, skills and professional experience, Summary of Job Function The Oncology Care Transition Coordinator is a registered professional nurse who under the supervision of the Oncology Care Transitions Manager contributes to the care of any hospitalized cancer patients by engaging patients in the transitional care management process and by collaborating with the inpatient and ambulatory clinicians, This person meets with patients upon admission to the inpatient unit, participates in multidisciplinary daily rounds, ensures the coordination and confirmation of appointments, follow up requirements, and needs for discharge,

Job Qualifications/Education/Training/Certification/Licensure Education:

Baccalaureate degree or higher Graduate of an accredited school of nursing Currently licensed to practice professional Nursing in the State of New Jersey BLS Certification Required Previous oncology experience and OCN preferred RWJBarnabas Health is an Equal Opportunity Employer
