As an RN Care Manager , you’ll provide both hands-on nursing and care coordination, playing a key role in delivering personalized, team-based care.

Key Responsibilities:

Deliver nursing care across clinic, home, and facility settings Lead care planning as part of an interdisciplinary team Monitor chronic and acute conditions, manage referrals, discharges, and medications Educate participants and caregivers on health, safety, and medication Participate in nurse triage rotation and IDT meetings Supervise home health aides and support staff

Qualifications:

Active RN license (NJ); BSN preferred Minimum 1 year of nursing experience (home care, LTC, or managed care a plus) Passion for working with frail or elderly adults Strong critical thinking, communication, and collaboration skills Proficiency with EMRs and Microsoft Office Valid driver’s license and reliable transportation
