Per Diem Rate:

$40 – $55/hour (Based on experience and case complexity)

Client Assessments:

Conduct comprehensive in-home evaluations to create tailored care plans, ensuring compliance with all state guidelines.

Personalized Case Management:

Coordinate services and communicate effectively with clients, families, and healthcare partners, maintaining meticulous records.

Team Leadership:

Supervise, train, and support our direct care employees in client homes, ensuring adherence to best practices and HCSF standards.

Quality Assurance:

Monitor care plans, maintain accurate documentation, and drive continuous quality improvement.

Who We’re Looking For:

Qualifications

• Active New Jersey Registered Nurse (RN) License in good standing.

You’ll need to have graduated from an accredited nursing program (ADN or BSN required) and have successfully passed the

NCLEX-RN.

• Exceptional clinical judgment and strong leadership skills (prior supervisory experience preferred).
• Ability to work independently, exercise sound discretion, and excel at problem-solving.
• Outstanding communication and interpersonal skills.
• Valid driver’s license, proper auto insurance, and willingness to travel throughout our New Jersey service area of Monmouth and Ocean counties.
• Maintains professional malpractice insurance and is committed to fulfilling biennial license renewal requirements, including continuing education and background checks.
