Collects healthhistory data from patient/family/significant other, and fromprevious medical records, while interacting with patient based onadolescent, adult or geriatric principles of communication.
Completes admission assessment within the acceptable time frame.
Notifies consults when admitting criteria has been identified.
Completes Braden scale appropriately. Continually reassesses thedevelopmental biophysical, psychosocial, environmental, self-care,educational, and discharge needs of patients. Createsindividualized age appropriate plan of care by involving patientfamily significant other, reflecting the need for safety,education, and a healing atmosphere. Updates and revises the planof care as needed and as patient status changes, in consultationwith patient, family and caregiver. Develops realisticoutcomes/resolutions based on developmental stage in life,integrating into practice the physiological changes andpsychological needs related to the elderly (i. E., skin integrity,safety, depression). Recognizes and documents interventions neededto obtain outcomes/resolutions. Continually reevaluates patients’needs accurately, initiating appropriate nursing actions to addressneeds. Delegates responsibilities for care, when applicable, andmakes assignments according to skill level of care giver. Performspatient care activities based on priorities of care. Integrates,delegates and coordinates patient care using an interdisciplinaryapproach.
- Documentation reflects the patient’s response tonursing interventions, medications, treatments and education whilecorresponding to age appropriate expectations.
Documentation islegible, timely, accurate and in accordance with policy andprocedure. Complies with Department of Health and other regulatoryagency requirements. Updates bed boards and communication ofbedsides needs. Assesses patients/family’s/significant otherscomprehension, skill level and barriers to learning. Initiates andcoordinates patient/family/significant other teaching utilizing ageappropriate instructions and teaching tools to reinforce areas ofknowledge deficit. Provides teaching bag to each patient, reviewingits contents upon unit admission. Documents all teaching performedto both patients/families on interdisciplinary teaching record.
- Discharge plan is initiated at the time of admission andimplemented throughout the hospital length of stay.
Participates ininterdisciplinary patient care meetings. Develops, initiates, andparticipates in the process of patient education, dischargeinstructions, and preparation of patient/family/significant otherfor discharge. Continues education and reinforcement of patientdischarge plan with all involved. Accomplishes a timely,comfortable, and safe discharge.
- Transcribes orders correctly.
Signs off orders, using date, time, 1st initial, full last name andstatus correctly and in a timely manner. Utilizes Computer system.
Accepts phone orders as per policy and documents readback.
Clarifies unclear, illegible, or non-specific orders, prior toimplementation.
- Other duties may be assigned.
Applies telemetryleads correctly for monitoring; addresses the purpose of cardiacmonitoring with the patient family significant other. Coordinateswith the Telemetry Technician individualized arrhythmic parametersfor the patient’s condition to ensure correct alarm settings areset. Interprets and documents telemetry monitoring stripsaccurately keeping attending Physician informed of ectopy.
Initiates chest pain protocol as indicated utilizing appropriatenursing interventions and documenting outcomes. Follows telemetrystanding orders. Manages patients on high risk intravenous cardiacdrugs and understands the implications of treatment. Assists withelective cardioversion

Required Licenses:
Valid NJ RNLicense Qualifications/Requirements:
- BLS Required
- ACLS Required
PCCN Preferred Relevant Experience:
- 1-2 years of experience inCardiac/Telemetry
- 1-2 years in Acute Care
- Basic dysrhythmiaexperience required
Education:
BSN Preferred or enrolled in BSNprogram
