Responsibilities
Conduct comprehensive patient assessments and reassessments.
Manage and track case progress, ensuring timely follow-ups and necessary assessments.
Maintain thorough documentation and stay on top of assessment dates.
Provide high-quality care and support to patients.
Communicate effectively with patients, families, and the homecare team.
Requirements
Valid RN license in New Jersey
CPR and First Aid certification (required)
Experince with HomeCare
Reliable car and a valid driver’s license
Willingness to travel throughout New Jersey
Strong case management skills with the ability to manage multiple patients
Excellent organizational skills and attention to detail
