PayInformation $2,166 per week About The Position The Registered Nurse(RN) – MDS Coordinator is responsible for coordinating thecompletion of the Minimum Data Set (MDS) assessments for residentsin long-term care or skilled nursing facilities. This role involvesassessing, collecting, and analyzing resident data to ensureaccurate and compliant documentation for care planning,reimbursement, and regulatory requirements. The MDS Coordinatorplays a critical role in the care planning process and ensuringthat the facility meets all federal and state regulations relatedto long-term care.

Key Responsibilities :
MDS Assessment andCoordination :

Conduct comprehensive and accurate MDS assessmentsfor residents, ensuring all data collected meets the facility’sstandards and regulatory requirements. Ensure that assessments arecompleted timely (typically every 3, 6, or 12 months), andaccurately reflect the resident’s current health status, includingmedical conditions, functional abilities, and psychosocial factors.

Work with interdisciplinary team members (e.g., physicians, socialworkers, therapists) to gather relevant information for the MDSassessment. Update MDS assessments as needed based on changes in aresident’s condition, care plan, or regulatory requirements.

CarePlanning and Collaboration :

Collaborate with the care planningteam to develop individualized care plans based on the MDSassessments, ensuring that residents’ needs are met and addressedappropriately. Ensure care plans are reviewed and updated asneeded, based on ongoing MDS assessments, changes in the resident’scondition, or feedback from the care team. Provide support inimplementing and monitoring care plan goals, ensuring consistencyacross various disciplines.

Regulatory Compliance :

Maintain anin-depth knowledge of federal and state regulations related to MDSassessments and long-term care documentation. Ensure the facilityremains in compliance with Centers for Medicare and MedicaidServices (CMS) guidelines, state regulations , and The JointCommission (TJC) standards regarding the MDS process and caredocumentation. Ensure timely submission of MDS assessments to theappropriate regulatory bodies for reimbursement and qualitymeasures.

Documentation and Reporting :

Accurately document all MDSassessments, care plans, and updates in the electronic healthrecord (EHR) or MDS software systems . Generate and submit requiredreports for Medicare , Medicaid , or other payer programs, ensuringproper billing and reimbursement. Maintain up-to-date documentationregarding each resident’s progress, care plan changes, and anymodifications in their health status.

Quality Assurance andMonitoring :

Perform regular audits and reviews of MDS assessmentsto ensure that they are completed correctly, fully, and incompliance with all applicable regulations. Monitor resident careoutcomes to ensure that services provided are effective and thatthe facility meets required quality measures for health careservices. Participate in facility quality improvement initiatives,particularly those involving care plans, documentation, and MDScompliance.

Training and Education :

Provide training and supportto nursing staff, care providers, and other team members on the MDSprocess and requirements. Educate staff on how to accuratelydocument resident care, complete MDS assessments, and useMDS-related software or systems. Stay updated on regulatorychanges, MDS guidelines, and best practices through ongoingeducation, professional development, and participation in relevanttraining.

Communication and Liaison :

Serve as the primary point ofcontact for MDS-related questions, concerns, or clarificationswithin the facility. Communicate effectively with residents,families, and the interdisciplinary team regarding assessmentoutcomes, care planning, and any necessary changes to residentcare. Work closely with facility leadership and externalstakeholders to ensure that all MDS assessments are conducted andprocessed according to regulations.

Resident and Family Advocacy :

Advocate for residents’ needs and rights, ensuring that the careplans and MDS assessments fully reflect the individual’spreferences, goals, and healthcare needs.
